Insurance Plans

We participate with many insurance and managed care plans. If your plan is not listed here, please call our office at (908) 851-2771 for updated information.

Many insurance companies utilize preferred labs and hospitals. Using non-preferred facilities may cause you to be solely responsible for the bill. Patients should check with their insurance company before scheduling any tests or procedures.

Referrals

Since many plans have specific requirements, please check with your insurance carrier to confirm whether or not a referral from your primary doctor is required prior to your visit. Our staff is here to provide any help or guidance you may need, however it is the patient's responsibility to obtain the required referral form from their primary care physician, keep track of the expiration of the referral, and obtain a renewal when necessary.

Payments

Payment is due at the time of your visit. If we are a participating provider in your insurance plan, payment of any deductible or co-payment will be due at the time of your visit.

Patient accounts are managed by Questcare Practice Management Services, Inc. If we are a participating provider in your insurance plan, Questcare will submit your claim for payment. Patients should contact Questcare directly with any billing questions by calling them toll free at (800) 784-5820.
